DESCRIPTION OF TRAINING PROGRAM
Clinical activities: The Mount Sinai Hemophilia Treatment Center (HTC) is the Health and Human Services Region II Grantee for the Maternal and Child Health and Centers for Disease Control (CDC) and Prevention funding. It offers comprehensive hemophilia care for approximately 250 patients with coagulation disorders, 75 of whom are HIV infected and 36 patients with vWD. The center also serves as a secondary and tertiary treatment center for consultation and surgery to patients from other metropolitan area hemophilia programs.
HTC staffing includes two hematologists (one pediatric, one adult), an orthopedist, oral surgeon, dentist, two nurse clinicians, one research nurse, two social workers, physical therapist and a dental hygienist. The HTC clinic utilizes 1,000 square feet of space with clinic sessions every Friday. This area includes a waiting area, nurses/phlebotomy station, staff room and seven examinatioon rooms. In excess of 750 clinic visits per year, both comprehensive and follow-up, are provided through these clinic facilities. The hemophilia program in collaboration with the Departments of Genetics and Obstetrics-Gynecology routinely provide carrier testing, genetic counseling for known carriers and pre-natal testing. The HTC participates in the greater New York Blood Program of the New York Blood Center and the World Health Organization-approved hemophilia training facility.
Administrative space for the HTC is provided in a 1,000-square-feet office suite. Staff offices, patient records, research data and a complete installation of network computers are housed in this area. Complete patient records are maintained in chart form and stored in the hemophilia centre's administrative offices. The staff of the HTC include an administrator, the region II coordinator, clinic coordinator, and two administrative assistants.
The Mount Sinai School of Medicine is the core center for both a CDC Cooperative Agreement and a Maternal Child Health Bureau Grant. The regional office is located within the HTC, housing the region II coordinator who works with our affiliates in NY, NJ and PA, 12 treatment centres in total. The training of new physicians and staff is coordinate by the core center.
Laboratory activities/research: Because of its large, stable, compliant patient population, the quality of the institution’s coagulation laboratory and close collaborative relationship with the Mount Sinai AIDS Clinical Trial Unit, the clinical research component of the HTC has gained a worldwide reputation. The Mount Sinai HTC continues to participate in research sponsored by US National Institutes of Health (NIH), Centers for Disease Control (CDC), and industry-sponsored studies. It is the study site for over 25 protocols including phase II safety and efficacy trials of new factor concentrates, therapeutic trials, blood safety surveillance, and prospective natural history studies of HIV infection.
Multidisciplinary activities/research: Clinical services include a diagnostic coagulation laboratory, emergency facilities, and in-patient services for hemophiliacs, 24-hour hematology, orthopedic, neurology, dentistry and oral surgery coverage and efficient utilization of the hospital blood bank. The centre has two nurse clinicians, two social workers, a physical therapist and a dental technician. Pro bono physicians provide orthopedics and infectious disease services during clinic.
Additional information: WFH fellows are offered the opportunity to train, not only at Mount Sinai, but also at several other affiliate locations when funding permits. Past trainees have had positive experiences at the comprehensive centers at Cornell, Long Island Jewish Medical Center, Robert Wood Johnson School of Medicine, the Mary M. Gooley Hemophilia Center, the United Health Services Hospital Blood Disorder Centere and the University of Puerto Rico. The centre can train two fellows annually, year round except for June and July.
